重磅官宣 | 如意珍宝丸正式纳入《高尿酸血症及相关病中医诊疗指南》

高尿酸血症并非单纯血尿酸升高,而是多代谢紊乱共同作用的结果。其临床结局可累及多系统、多器官。

《高尿酸血症及其相关病中医诊疗指南》是由中国中医科学院广安门医院和青岛大学医学院作为共同牵头单位,基于临床证据、古籍文献、多学科专家共识编制而成。本指南围绕高尿酸血症展开,主要梳理了高尿酸血症及其相关疾病的中医诊断要点与治疗方案。内容涵盖高尿酸血症、痛风性关节炎、痛风石病变、痛风性肾病,以及高尿酸血症合并肥胖、高脂血症、高血压、脂肪肝、2型糖尿病、心脑血管病在内的共10种相关疾病。旨在为高尿酸血症及其相关疾病提供以控制血尿酸为核心的综合治疗方案,提升我国高尿酸血症的中医综合诊疗水平。

尿酸血症

高尿酸血症(HUA)是指机体嘌呤代谢紊乱,尿酸分泌过多或肾脏排泄功能障碍,导致尿酸在血液中积聚的病理状态。当血尿酸浓度超过其在血液或组织液中的饱和度后,可在关节局部形成单钠尿酸盐(MSU)结晶并沉积,进而诱发局部炎症反应与组织破坏,即引发痛风。

我国针对高尿酸血症和痛风开展的大规模的流行病学调查报告显示[1-4],中国大陆普通人群的高尿酸血症患病率为17.4%(男性为22.7%,女性为11.0%),痛风患病率为1.6%(男性为1.9%,女性为0.5%),患病率随年龄增加而增加,男性高于女性,沿海高于内陆,城市高于农村的特点。

高尿酸血症危害

高尿酸血症的危害远不止引发关节和肢体疼痛变形,还会诱发多种难以逆转的慢性疾病。临床与基础研究表明,高尿酸血症的危害远超痛风性关节炎,还累及代谢、心血管、肾脏及心理健康等多个系统,显著增加患者并发症风险与全因死亡率。高尿酸血症和痛风与肥胖[5]、高血压[5]、糖尿病[6]及心脑血管疾病[7]相互影响。其中肥胖,尤其是内脏脂肪增多导致的腹型肥胖,与高尿酸血症密切相关[8]

研究显示,尿酸水平升高导致内皮细胞损伤、血管炎症,加速动脉粥样硬化进展,进而促进高血压的发生[9-10]。血尿酸水平增高会导致2型糖尿病患者罹患心脑血管疾病的风险[11-13]。血尿酸主要通过线粒体氧化应激,促进肝脏脂肪合成与积累。而线粒体氧化应激通过抑制三羧酸循环中丙酮酸酶的活性,从而增加肝细胞中的脂肪沉积与合成[14]
